    Medication:
    Medication administration in a home health setting involves safely assisting clients with their prescribed medications according to their care plan, physician’s instructions, and state regulations. This support ensures clients receive the correct dosage at the right time and helps avoid missed or incorrect medications, which can lead to serious health complications.
    Key Responsibilities:

    • Medication Reminders:
      Prompt and encourage clients to take medications as prescribed.
    • Medication Assistance (where allowed):
      Assist with opening containers, handing medications, and providing water, ensuring medications are taken properly.
    • Medication Administration (by licensed personnel):
      Administer oral, topical, inhaled, or injectable medications as prescribed, following physician’s orders and proper protocols.
    • Monitoring:
      Observe for side effects, allergic reactions, or changes in client behavior and promptly report concerns to the nurse or supervisor.
    • Documentation:
      Accurately record medication administration times, dosages, and any client responses or issues in the client’s medical record or MAR (Medication Administration Record).
    • Medication Storage & Management:
      Ensure medications are stored safely and in accordance with safety guidelines (e.g., locked away from children or unauthorized individuals).
    • Coordination with Healthcare Providers:
      Communicate with doctors, pharmacists, and nurses regarding medication changes, refills, or concerns.

    Important Notes:

    • Unlicensed staff may only remind or assist with self-administration depending on state regulations.
    • Licensed nurses (LPNs, RNs) may perform full medication administration and injections.
    • All personnel must follow HIPAA and maintain client confidentiality at all times.

    Meal Preparation:
    Our meal preparation service ensures that clients receive nutritious, well-balanced meals tailored to their dietary needs and preferences. This support promotes health, independence, and quality of life, especially for those with limited mobility, medical conditions, or cognitive challenges.
    Duties Include:

    • Planning simple, nutritious meals based on the client’s dietary restrictions (e.g., diabetic, low sodium, soft foods).
    • Grocery shopping assistance or preparing meals from pre-stocked ingredients.
    • Preparing breakfast, lunch, dinner, and/or snacks as needed.
    • Ensuring food is safely stored and kitchen is cleaned after meal preparation.
    • Encouraging hydration and timely eating habits.
    • Monitoring food expiration dates and maintaining a clean, safe food environment.

    Goal:
    To support healthy eating habits while honoring client preferences and independence in a safe and sanitary home environment.
    Mobility and Transferring :
    Our caregivers are trained to assist clients safely and respectfully with mobility and transferring to help maintain independence and prevent injury.
    Services include:

    • Assistance with Walking: Supporting ambulation using gait belts, walkers, canes, or standby assistance as needed.
    • Bed Mobility: Helping clients reposition in bed, turn to prevent bedsores, and sit up safely.
    • Chair and Wheelchair Transfers: Assisting with transfers between bed, wheelchair, toilet, shower chair, or other surfaces using proper body mechanics and transfer techniques.
    • Use of Assistive Devices: Ensuring safe and correct use of mobility aids such as transfer boards, mechanical lifts, and mobility equipment.
    • Fall Prevention Support: Assessing and reducing fall risks within the home and providing steady assistance during movement.

    Light House Work:
    As part of our commitment to supporting a clean, safe, and comfortable home environment, our Light House Cleaning services assist clients with essential household upkeep that promotes health and well-being. These tasks are non-medical and designed to maintain a tidy living space, reduce fall risks, and support overall home hygiene.
    Services May Include:

    • Dusting furniture, shelves, and surfaces
    • Sweeping, vacuuming, and mopping floors
    • Cleaning and sanitizing bathrooms (toilets, sinks, counters)
